Executive Function Assessment: Why Traditional Approaches Miss the Bigger Picture

Traditional executive function assessments tell us what a child struggles with. But do they help us understand why those challenges occur, or where meaningful intervention should begin?




If you've ever assessed a child and thought, "Something isn't adding up," you're not alone.

Perhaps a student organizes materials independently in one classroom but appears completely overwhelmed in another. A child who follows multi-step directions at home struggles to begin simple tasks at school. Another shows excellent planning during therapy sessions but seems unable to use those same skills on the playground.


Was the assessment wrong?

Probably not.


But the interpretation may have been incomplete.


Many people describe executive function as a collection of cognitive skills, including planning, organization, working memory, inhibition, flexibility, and self-monitoring. While these skills are certainly important, assessing them in isolation rarely tells the whole story.


The real challenge is this: executive function is not equally accessible in every situation. A child's ability to plan, organize, shift attention, or regulate behavior can change dramatically depending on internal state, environmental demands, relationships, communication supports, and the expectations of the task.


Without considering these factors, we risk measuring performance instead of understanding participation.


Executive Function Is Dynamic, Not Fixed


Executive function isn't something a child simply "has" or "doesn't have." It changes throughout the day. A child may show excellent executive function while building with LEGO® but struggle to begin a writing assignment 30 minutes later. Another may independently manage a familiar morning routine at home but become disorganized in the busy, unpredictable environment of the classroom.


The differences don't indicate inconsistent effort or motivation. More often, they reflect changes in executive function access.


Access is influenced by many factors, including:


  • Regulation state

  • Interoceptive awareness

  • Fatigue

  • Sensory load

  • Environmental demands

  • Communication supports

  • Task complexity

  • Relationships and co-regulation

  • Demand-capacity fit


Understanding these influences helps us move beyond asking, "Can this child perform this skill?" and toward a more meaningful question: "Under what conditions does this child have access to executive function?"



Why Traditional Executive Function Assessments Sometimes Fall Short


Many executive function assessments do an excellent job of identifying areas of difficulty. They can tell us that a child struggles with working memory, planning, organization, or inhibition.


But they often leave clinicians asking another question:

"Now what?"


Knowing that a child has difficulty initiating tasks doesn't explain why task initiation changes from one environment to another. Knowing that working memory scores are low doesn't tell us whether sensory overload, emotional stress, communication demands, or regulation challenges are reducing access in everyday situations.


Executive function never develops independently of the body or the environment.

Children participate in relationships, classrooms, homes, playgrounds, therapy sessions, and communities. Every one of those contexts influences their executive function.



Looking Beneath the Surface


As occupational therapy practitioners and other pediatric professionals, we're trained to look beyond observable behaviors. When a child struggles to begin work, loses materials, appears impulsive, or becomes overwhelmed, those observations are important, but they're only the starting point.


Meaningful assessment asks deeper questions:


  • What supports are already helping?

  • When is executive function most available?

  • What environmental demands increase difficulty?

  • How does regulation influence participation?

  • Does the child recognize internal body signals?

  • Are communication demands affecting executive function?

  • What changes across settings?


When we understand the answers to these questions, interventions become more targeted, collaborative, and effective.


A Body-First Perspective on Executive Function


One of the most important lessons we've learned over the past decade is that executive function cannot be separated from the nervous system. Before children can consistently plan, organize, inhibit responses, or shift attention, they must first have access to the internal resources that support those skills.


Regulation matters.

Interoception matters.

Relationships matter.

Environmental fit matters.

Participation matters.


Rather than viewing executive function as a fixed cognitive ability, a body-first perspective recognizes that executive function emerges through the interaction of the brain, body, environment, and meaningful participation. This shift changes not only how we assess executive function but also how we support its development.
